The fourth edition of this book was published in 2024. This volume is designed to use on devices such as computers, tablets, or even smart phones. Purpose This book is serving its 33rd year as an essential resource supporting the management of critically ill adults. Emphasis is placed on fundamental content and concepts designed to be employed in virtually any intensive care unit (ICU) despite the initial description of the critical care unit. Presentations are intended to be concise and readily appreciated by the audience. References are updated with the inclusion of clinical practice guidelines connected with the various chapters. Remarkably, this book is the product of the academic work of a single author. Audience Virtually any discipline working in the ICU is part of the primary audience for this book. For example, intensivists from several home disciplines including internal medicine, surgery, anesthesiology, pharmacy, and respiratory therapy, along with administrators serving the unit are an appropriate audience for these chapters. Dr. Paul Marino, MD, PhD is a fellow of the American College of Critical Care Medicine and a clinical associate professor at the Weill Cornell Medical College in New York City. Features An attractive soft-bound work includes 53 chapters with three appendices and a subject index which comprises a total volume of over 800 pages. Chapters in this fifth edition are organized into 16 sections. In the course of 33 years of episodic preparation, this book has been completely rewritten with updated references and the inclusion of selected practice guidelines with each chapter. New chapters include fluid management, clinical aspects of shock including cardiogenic problems, acute pulmonary embolism, and an additional new chapter reviewing noninvasive ventilation. The book also indicates there are 238 new illustrations along with 207 freshly prepared tables along with summary comments to be included with each chapter. These summary comments, titled "A Final Word" are organized to allow for additional insight into the clinical issue of interest in the chapter. Three chapters in the first section of this widely used book describe vascular access beginning with the physics and physiology of providing adequate routes to the vascular bed of the patient. The specialized catheter for long-term vascular access is described along with specific techniques for utilization of the central veins. Section II provides three additional chapters describing prophylactic access techniques for the venous system along with a preventive approach for peptic ulcer disease and common strategies for analgesia and sedation in the ICU. Three related topics are next discussed including management of intravenous fluids, various blood products, and the related needs of the patient with various forms of shock. Next discussed are a variety of core respiratory topics including COPD, asthma, acute respiratory distress syndrome, and essential strategies for management of the mechanical ventilator. Subsequent chapters provide additional information regarding fluids and electrolytes while other presentations on abdominal organs review liver failure, pancreatitis, and abdominal infections. Concluding chapters describe various forms of nutrition and endocrine support, ICU pharmacology, and temperature control in the critical care Unit. Three final appendices summarize standard units of measurement, reference ranges, and a philosophical approach to providing care in the ICU. Chapters are clearly written with multicolor tables of clinical data and colored figures describing interrelation of practices in the unit. Multiple references are provided with subtitles related to chapter content. The table of contents lists chapters and describes sections with chapter groups while a detailed subject index includes separate citations for tables and figures. Assessment This book is a longstanding overview reference for use in the critical care unit. Employment of a single author provides a consistent message and format. This is an effective introduction for the many disciplines addressing life in the multidisciplinary critical care setting. Additional reference reading may be employed to obtain complimentary information for topics introduced here. Having grown up with this volume in prior years, I enjoyed reading this edition.
