This book describes the issues and challenges that clinicians encountered in the management of older critically ill patients during the Covid-19 pandemic, and offers practical information on how to manage them. Older adults are more susceptible to complications such as acute respiratory distress syndrome (ARDS) as a result of viral pneumonia. In addition, they often have multiple comorbidities and are commonly frail, which means their various organs and systems, such as the respiratory system, have reduced functional reserves. As such, older adults are less able to react to acute stressors. During the current Covid-19 pandemic, older adult patients mortality is increased. Further, the infection and death rates of elderly people in nursing homes and health care institutions are high.Management of older adults with Covid is complicated. The reduced availability of beds may limit their access to ICU. Moreover, the prognosis may be poor, and airway management and ventilation strategies have to take into account various clinical and physiological characteristics specific to older patients. This book is addressed to all allied professionals involved in the management of older critically ill patients and presents information collected and practical lessons learned from the clinical daily management of this population during the pandemic.

It also focuses on decision making, resource utilization, and how to appropriately manage older adult patients with COVID-19 associated respiratory failure. These are very important objectives, which the book addresses commendably. |

| The audience for this compilation is frontline healthcare workers, specifically hospitalist and critical care providers who care for older, more frail patients in the acute care setting. Critical care/pulmonary, geriatric medicine, and hospice/palliative care fellows and residents would also be a keen audience. Critical care nurses and respiratory therapists might also find the book of interest. The authors are credible, and the work is appropriately referenced for the proposed audience. |

The bulk of the compilation is focused on ICU management of older adult patients with COVID-19 pneumonia and acute renal failure. Lastly, the book addresses resource utilization and decision making around this concern during a pandemic. |

Assessment

| This book is a worthwhile read. It is unique in its focus and perspective and very much needed. One concern is that because the evidence and recommendations are moving so quickly during the pandemic, some of the more specific recommendations may be out of date quickly. Despite this concern, the book is of good use and should be considered for library purchase. It is well done with good illustrations and radiographic examples. Other than concerns about the rapidly evolving nature of the pandemic recommendations, this reviewer would have liked more focus on COVID-19 associated encephalopathy and delirium, which has been a significant issue complicating the management of critically ill older patients and has further complicated post-ICU outcomes for older adult patients. This is by far the most significant omission in an otherwise well-organized and inclusive compilation. Additionally, the inclusion of a geriatric screening tool that utilizes the Mini-Mental State Exam (MMSE) is an oversight, likely because the tool has been copyrighted. More importantly, the book has a very distinct western and English as a native language bias and misses non-Alzheimer's cognitive disorders. Despite these concerns, this book is a worthy project that was done in a timely manner and is a good addition to the field. |
