Description
This is the fifth edition of a manual describing the essentials of trauma care that originates from the University of Pittsburgh. With a greater emphasis on emergency general surgery, the manual has increased in size, to 75 chapters and approximately 1,000 pages.
Purpose
Coverage includes fundamentals of management of injury and related critical care. A prominent addition to this manual is multiple chapters on the fundamentals of emergency general surgery. Brief chapters written by experts are a refresher for more senior providers and a welcome, quick introduction for new practitioners.
Audience
Surgeons, intensivists, and emergency physicians at all levels of training will benefit from this book. As was the case in previous editions, many of the authors are from programs at the University of Pittsburgh. Increased input from national and international experts has also been sought.
Features
An opening group of chapters describing general considerations review unifying topics in emergency surgical care such as shock, the approach to damage control, wound healing, mass casualty events, relevant imaging, pain management, palliative care, and surgical emergencies at the extremes of age. A second group of chapters focus on trauma based on body region beginning with an overview of mechanism of injury. Orthopedic, neurological, vascular, and genitourinary trauma are included. A third group of chapters present critical care based on organ system support with a particular emphasis on cardiopulmonary critical care and multiple organ dysfunction. Acute general surgical problems are included in a fourth large group of chapters. Presentations feature management of the acute abdomen, gastrointestinal bleeding, bowel obstruction, hepatobiliary disease, inflammatory bowel disease, anorectal conditions, vascular emergencies, hernias, and obstetrical problems. A brief concluding group of chapters reviews complications of bedside procedures and abdominal and thoracic trauma. Two appendixes include scoring systems for organ injury and emergency general surgery from the American Association for the Surgery of Trauma. A subject index includes separate citations for figures and tables. Chapters are written in outline form. Multicolor type highlights division of subheadings. Multiple tables emphasize important data, clinical algorithms, and details such as scoring systems. Black-and-white radiographs and photographs reproduce with adequate quality; color drawings are superb. The color photographs also show well. Chapters conclude with detailed reference lists featuring important primary literature. The table of contents describes chapter organization and attributes authorship.
Assessment
This edition is dramatically enhanced from the previous 2013 edition. The quality of the text production is dramatically improved, and the breadth of content is excellent. A significant number of the additional authors are national and international experts. While the manual has grown beyond the size easily carried in one's pocket, this rapid and elegant reference is worthy of the bookshelf of the emergency surgeon.
