The resources, or tools, are described along with examples of practical application and evidence of success. Purpose The purpose is to present strategies for applying evidence-based practices to effectively deal with the ongoing problem of pressure ulcers. Because pressure ulcers continue to be a costly problem in terms of patient well-being and money, The Joint Commission included pressure ulcer prevention in its National Patient Safely Goals. A project was carried out from 2009-2011, led by Nurse Safety Scholars-in-Residence. This toolkit is the result of the project. Audience This is a resource for any healthcare provider interested in and tasked with the care of persons at risk for developing pressure ulcers. The language is geared to professionals, but with the excellent illustrations and CD, the message can easily be shared with the nonprofessional caregiver. The book does an excellent job of including tools and resources from very credible authorities in the field. Features The toolkit promotes evidence-based practice. It is a practical guide to setting up a system for identifying risk, planning interventions, and sustaining the program. The book discusses the rationale for developing systems that are suited to a variety of settings and cultures and presents examples of success. The CD uses pictures and video clips to teach the different components of a successful program. The CD is in color, but the book is black and white, perhaps its only shortcoming. Assessment This is an excellent resource because of its many credible tools. Readers can determine pros and cons and what would work in their particular setting. The CD provides a visually stimulating presentation and valuable resource for education. The success stories are real-life examples illustrating and validating the benefit of implementing a system.
