Description
This is a valuable reference for pharmacy personnel who engage in the practice of nonsterile compounding. The authors performed a thorough literature review in order to include over 300 compounding formulas for approximately 200 medications in this fourth edition. This edition includes 129 new records when compared to the previous edition published six years ago. The organization of the book is very user-friendly. There are several sections including compounding formulas for oral solutions, suspensions, or syrups; compounding formulas for topical or ophthalmic use; compounding formulas for commercially available products; and a few appendices. The compounding formulas are easy to read and visually appealing. Each page includes a clear description of the compounding materials needed, procedures, labeling, and stability information. The reference for the compounding formula is conveniently located at the bottom of the page. Medications on the NIOSH hazardous drugs list are also clearly indicated in a black box near the top of the compounding formula. The introduction and appendices to the book contain helpful information about compounding methods and techniques, legal implications of pharmaceutical compounding, and USP <795> requirements.
Purpose
According to the preface, the book is intended to be a trusted resource for pharmacy personnel in the preparation of extemporaneous formulations. This book meets its objective through the inclusion of numerous compounding formulas in various forms and concentrations for hundreds of medications, and information about compounding requirements and techniques. The authors have been scrupulous in their analysis of published literature and selection of studies, describing an exhaustive literature search and inclusion of only formulations with recognized stability data. There are some major changes in this fourth edition when compared to the previous third edition. From a content perspective, there are 129 added compounding formulas and 14 deleted compounding formulas. The authors also revised the format of the compounding formulas in this edition to include information that is expected to be required in the upcoming release of the USP <795> update, which include six new fields: Equipment and Supplies, Quality-Control Procedures, Labeling Requirements, Study Container Type, Referenced Manufacturers, and Stability-Indicating Study.
Audience
This book is intended for use by any pharmacy personnel involved in the compounding of extemporaneous formulations to meet the needs of special patient populations. The target audience could be clinicians practicing in an institutional environment or a community pharmacy setting. The authors do note that this book is not meant to serve as a reference for pharmaceutical manufacturers. I believe this is appropriate, as pharmacists who compound medication formulations on a small scale are not held to the same standards and requirements as large-scale manufacturers. The four authors appear to be credible authorities on the subject of compounding extemporaneous formulations for special populations, as two of the authors are employed by the Children's Hospital of Pennsylvania (CHOP.) Another author is employed by the Institute for Safe Medication Practices (ISMP) and thus could be viewed as an expert in safe compounding methods and techniques.
Features
This book is a valuable reference for pharmacy personnel who engage in the practice of nonsterile compounding. This fourth edition contains hundreds of compounding formulas, sourced from references with documented stability. Thus, practitioners can be confident in the source of information when utilizing this book to support nonsterile compounding practices. One of the successes of this book is the organization. The design of the book, featuring easy-to-follow compounding formulas and several appendices including useful information about compounding methods and technique, is effective. One such appendix is particularly helpful, with information about compounding requirements that are expected to be included in the upcoming USP <795> update. Another advantageous appendix is the list of standardized concentrations from ASHP's Standardize 4 Safety list of compounded oral liquid formulations. A major portion of the formulations in the book are for medications that also have commercially available liquid formulations. The authors indicate that these compounding formulas are to be used if the commercially manufactured preparation is unavailable. Some may view the inclusion of compounding formulas for so many commercially available preparations as a shortcoming; however, in today's age of constant drug shortages and manufacturer backorders, this section can be beneficial.
Assessment
This book is one of the few titles of which I am aware that is a compilation of compounding information for formulations of medications for special patient populations. Over 100 formulations have been added to this new edition, so the updated version is necessary.
