Purpose The guidance this book provides includes an outline of the standards and how to meet them. In addition to sample logs and documentation forms, it provides tools for hospital pharmacies to grade themselves on meeting the medication management standards. Audience Intended for pharmacies to help them meet medication management standards, the guide also can be used by someone outside the pharmacy who has responsibility for accreditation. Features All the medication management standards are detailed, along with helpful examples of how a pharmacy can meet them. The guide also has a CD-ROM that can be uploaded to provide an online reference tool. The only shortcoming of this book is that the Joint Commission also provides online references with the same tools and they are more comprehensive, covering standards beyond medication management. Assessment This is a helpful tool. For pharmacy managers or pharmacists responsible for meeting accreditation standards for medication management, it provides useful information that is easy to follow.
