Description
The second edition, last published in 2007, describes how DBT (dialectical behavioral therapy) is applied to various disorders (PTSD, eating disorders, childhood disorders) and in different settings (inpatient, outpatient, school, corrections). It also addresses working with adolescents and families. Finally, there is information on training clinicians and pharmacotherapy.
Purpose
The purpose is to "[offer] key insights on implementing dialectical behavioral therapy (DBT) in real-world, clinical practice." According to the book, "this influential work has been substantially revised with 60% new material reflecting over a dozen years of research and clinical advances." These are worthy objectives that have been met.
Audience
According to the book, "highly practical and accessible, this important book belongs on the desks of all clinicians interested in DBT, including clinical psychologists, psychiatrists, clinical social workers, counselors, and psychiatric nurses. Graduate students and trainees in these fields will find it an invaluable text." I agree with the target audience described and the book meets the audience needs. The editors are credible authorities in the subject matter of the book. Linda A. Dimeff is chief scientific officer at Jaspr Health, institute director at Portland DBT Institute, and on the faculty at the University of Washington. She has collaborated with Marsha M. Linehan to develop programs with individuals who are dually diagnosed (BPD and substance abuse). Dr. Dimeff has won awards for her work and has over 55 publications. Shireen L. Rizvi is associate professor at Rutgers University. She is interested in DBT outcomes and training. She also has won awards for her work. Kelly Koerner is CEO of Jaspr Health and on the faculty of the University of Washington. She is interested in evidence-based practices implemented in diverse settings ("About the Editors," pp. v-vi). The other contributing authors are from the United States and Canada, affiliated with well-known institutions of higher learning and/or in private practice.
Features
The book begins with an introduction to DBT. It was developed by Marsha Linehan in 1993 to address the needs of chronically suicidal individuals. It utilizes behavioral principles to help decrease maladaptive behaviors. The authors encourage clinical settings to adopt the standard model of DBT as opposed to substantial modifications. Once an agency begins a DBT program, there should be an evaluation process that balances clinical outcomes and costs. Part II explores how DBT can be applied in different settings. Implementing an outpatient DBT program can involve changing agency policies and procedures. Research has shown that C-DBT programs in schools have decreased self-injurious behaviors. DBT is also effective in juvenile justice programs. In part III, there is information on the use of DBT across different populations. DBT may be modified for dually diagnosed clients (BPD and SUD). When applying DBT principles with adolescents, there can be significant difficulties, especially in light of potential self-injury. The book ends with a discussion of training and supervision of clinicians who are learning DBT. The book is very readable and contains figures and tables that help clarify the text.
Assessment
This book is excellent because it provides an overview of DBT and its clinical application across settings and populations. It is written by experts in the field. The second edition justifies replacing the first edition because the chapters have been updated, along with the addition of new material.
