Description
After an 11-year hiatus, the sixth edition of this neonatology book is finally out. Dr. Klaus has assumed an emeritus position in editorship and his role has been capably filled by Dr. Avroy Fanaroff's son, Jonathan. The unique qualities introduced in the first edition in 1973 have been continued in all subsequent editions, including editorial comments, questions, and case material throughout. The cases are not used to test readers, but to present new information and illustrate certain principles of direct patient care. Editorial commentaries supplied by Doctors Stern, Odell, and Prod'hom in the first edition are now supplied by the editors with a few exceptions.
Purpose
The purpose is present an up-to-date review of neonatology by many of the respected leaders in the field. Although not as comprehensive as the major textbooks, this book is concise and clinically oriented for house officers, nurses, or fellows. By limiting the scope, the editors cover the vast majority of major clinical issues in the field. In this rapidly changing area, the new edition is welcome and well worth the wait.
Audience
When the first edition of this book was published in 1973, general pediatricians still provided significant care to ill newborns. This book was perfect for their use. Today, the book is aimed at trainees and young staff, both physicians and nurses, who work in NICUs. Using case presentations holds readers' interest and challenges them with cases of varying levels of complexity. Editorial comments help to bring perspective to a field which still has large gaps of knowledge and controversy. The senior editor, Dr. Avroy Fanaroff, is the dean in this field in this country. Most of the contributing authors are recognizable experts, with a number of young authors adding some fresh perspective.
Features
Most of the 20 chapters are organ system-based, but requisite chapters on physical examination, the physical environment, resuscitation, stabilization and transport, and ethics are included. This edition has a new chapter on evidence-based medicine and the role of networks, as well as another revision of the very unique chapter presented in the first edition, "Care of the Parents." Despite some attention to dysmorphology in the chapter on physical examination, one shortcoming is the lack of guidance for young practitioners in genetics, dysmorphology (with some good illustrations), and inborn errors of metabolism.
Assessment
This is an iconic book in neonatology. Its reappearance after an 11-year absence is very welcome, indeed. Although not as inclusive as the major neonatal textbooks, it continues to be innovative in the presentation of material and the introduction of new subjects, such as the role of networks in providing evidence-based data. This book should be part of every neonatal library and this edition will find a place on my shelf next to the previous five.
